In re: Community Health Systems Inc. Customer Data Security Breach Litigation, Case No. 2:15-cv-00222-KOB

Who’s Eligible

Class Members include those whose personal information was exposed in the 2014 Community Health Systems data breach.

“The Settlement defines ‘Personal Information’ as information that is or could be used, whether on its own or in combination with other information, to identify, locate or contact a person, including without limitation names, addresses, birthdates, social security numbers, telephone numbers, and the names of employers and/or guarantors.”

Potential Award

$250 to $5,250 depending on the Class Member’s expenses and losses attributed to the Community Health Services data breach.

Up to $250 in reimbursement may be claimed for expenses related to cost of credit monitoring, time spent dealing with the data breach, cost of credit reports, payments for credit freezes, overdraft fees, phone calls, etc.

Up to $5,000 is available to Class Members who can provide documented monetary loss due to the security breach.

Class Members have the option of filing a claim for either or both of these payments.

Proof of Purchase

Class Members will need to provide proof of each claim, including proof of purchase of credit monitoring, time spent, and any other costs. Class Members making a claim for fraud will need to provide documentation of unreimbursed costs related to the breach.

Remember: you are submitting your claim under penalty of perjury. You are also harming other eligible Class Members by submitting a fraudulent claim.
